Admission Process at No Place Like Home Health (Part Two)
Assessments – Within 48 hours
Did you know that No Place Like Home Health is a 24 hour a day operation? That means that there are times that loved ones or someone that needs assistance with some form of care are researching or even inquiring about services that we can provide. We provide services 24 hours a day, seven days a week, 365 days a year (except leap year when we add another day). So, we take it seriously when someone contacts us about helping themselves or a loved one find care.
There are many ways to contact No Place Like Home Health as well.
Whether it is through our website, a phone call, email, friend, family, a partnered agency or facility we are always on call to help with your needs. Once you reach out to us we try to respond immediately and that is our goal. Sometimes our admission staff is tied up with other clients or with our caregivers. If that is the case you can expect a phone call back soon thereafter, usually within an hour. If it is after hours we will reach out the next morning. If it just cannot wait, please do not hesitate to tell us it is imperative that we contact you immediately and we will.
An assessment is our way to get to know our clients and understand the care that they will need. Within 48 hours we will schedule a free in-home assessment or come to whatever facility your loved one or yourself are currently in. During this time, we discuss the care options available and what to expect from our agency and caregivers.
Areas that are covered during our assessments are:
• Care needs
o Dementia
o Hospice
o General
o Companion
o Incontinence
o Etcetera
• Demographics
• Medical Conditions
• Activities of Daily Living (ADLs)
o Ambulation and transfers
o Bathing
o Continence
o Dressing and grooming
o Eating
o Toileting
• Instrumental Activities of Daily Living (IADLs)
o Housekeeping
o Meal preparation
o Medication reminders
o Other activities
• Daily routines and activities
These are some of the topics we cover in order to get an understanding what is needed for our clients and from our caregivers. Understanding these topics ensures that you get the proper care needed. We also complete a home safety assessment. Only after getting an understanding of the overall picture can we create our care plan.
